| The HCBW program provides an alternative to nursing home placement for patients who qualify for these services. Under this program, Home Health Aides are able to provide personal care services, homemaker services including household maintenance, assistance with laundry and cleaning, meal preparation, and respite services which allow the patient's primary caregiver time away while leaving the patient in the home with adequate supervision. In addition to the services, minor home adaptation may be provided as needed. The patient is assessed by the Social Worker who may also provide case management to coordinate the needed services. |
